Dr. Grein is a graduate of Indiana University School of Medicine, where he completed his residency in Pediatrics and his fellowship in Neonatal – Perinatal Medicine. He is a Pediatrician and Newborn Hospitalist at AdventHealth Hendersonville, taking care of our youngest and most vulnerable. We have developed a program unique to Asheville that marries medicine and medical oversight with lifestyle coaching, fitness, nutrition and body composition analysis to give our clients the best chance of lasting, healthy weight loss. The program is suitable for adults who are committed to making lifestyle changes and have a BMI that indicates they need to lose weight for health reasons (typically 25 – 40). We require clearance from your primary care provider prior to acceptance.
We use compounded semaglutide and tirzepatide, which are glucagon-like peptide-1 (GLP-1) receptor agonists. They were developed to help control blood sugar in Type – 2 Diabetes, and a side effect was appetite suppression, at different doses. Both of these medications have been studied and approved by the FDA for chronic weight management in individuals that are overweight and have chronic health conditions (diabetes, heart disease, hypertension, obstructive sleep apnea, or dyslipidemia).
Compounded medication is used in the place of manufacturer-produced medication for several reasons. It can be customized for our unique needs, it comes at a significant cost savings, it is produced in an FDA-inspected and approved pharmacy, and it allows us to ensure availability when supply is limited of the standard medication.
We require you to obtain lab studies from your primary care physician prior to starting our program (and within the last year) to ensure we document your health and to ensure you are suitable for the medication and the program. We also want to encourage a close relationship between our clients and their primary care physician during their weight loss journey.
We require the following labs: a complete metabolic panel (CMP), a complete blood count (CBC), a lipid panel, amylase, lipase, thyroid studies (T4, TSH), and HgbA1C.
The medication and lifestyle changes that are part of the program will take time to see lasting results. The first month of medication is at a minimum dose to ensure no side effects an to assess tolerance. During that time, you will continue to see us every week for medication and review, and you will be given a workout plan that is safe to start to encourage movement and getting your body ready to move. After the initial month, the medication is titrated up to effect and to balance side effects, and you will begin to see weight loss. You will also begin to work with our personal trainer on a personalized fitness plan to help achieve your goals.
The private Facebook group is designed to provide community support for our clients during their weight loss journey. We will have activities that are open to members announced through the group that will include walks or hikes and other meet ups. We will also share inspiration, recipes and fitness tips through the group. And finally, members will help members as we all work together to achieve our goals.

The Wellness Shot is a micro-dose of a GLP-1 agonist, well below the therapeutic level used for significant weight loss in the clinic. It is designed for people looking for the beneficial effects of the GLP-1 agonist peptide but who do no have a need to lose a significant amount of weight. You can expect to lose a small amount of weight on this dose, but it is designed for the additional benefits, such as menstrual regularity, cardiovascular health, reduced inflammation, and improved mental health.
